Dr. William Livingston – Costa Mesa Treatment Center

Dr. William Livingston became part of the Boston Medical Group in 2006 as the chief physician of the Costa Mesa, CA office. He obtained his undergraduate degree from DSU, he attended the University of Mississippi, and at the University of Pennsylvania where he got his medical degree. He was part of the United States Navy and Medical Corp. and worked for numerous naval hospitals. He served as Chief of Urology in the US Naval Hospital in Yokozuna, Japan.

Erectile Dysfunction

Erectile Dysfunction (ED) is the incapacity to maintain and achieve a sufficient erection satisfactory sexual intercourse. While it is mostly associated with older men, ED is a usual problem for men of all ages.

ED Causes

Psychological

Psychological factors include nervousness, performance anxiety, or fear of failure during lovemaking and account for about 10% of erection problems. These factors cause a surge of adrenaline, reducing the flow of blood to the penile area. Hence erectile difficulty.

Psychological Erectile Dysfunction can become self-perpetuating. After some incidents of repeated failure, the body becomes used to releasing adrenaline just at the thought of sexual encounter.

Other less usual causes include low sexual drive, guilt, stress, anxiety, sexual boredom or depression.

Physical

The remaining 90% accounts for physical factors categorized as vascular (relating to blood flow) and non-vascular diseases. It had been estimated that about 70% of all recorded ED cases could be attributable to vascular diseases alone.

To become fully erect, the penis requires a healthy blood flow. Even a minimal reduction can potentially cause problems.

Vascular insufficiency is associated with or caused by diabetes, hypertension, high blood cholesterol, cardiac diseases, smoking, and reduced overall circulation.

ED Adverse Effects

Erectile dysfunction can cause mental problems like feelings of inadequacy, frustration, poor motivation, denial and low self-esteem, and in worst cases, depression.

ED Symptoms

According to statistics, 52% of men over the age of 40 experience erectile dysfunction. Symptoms may vary but here is a list of the most common ones:
● Difficulty in maintaining an erection at least once in every four sexual intercourse attempts, or persisting for more than a month.
● Attaining an erection takes longer than usual, or becomes more difficult.
● Sexual erection becomes less rigid or weaker.
● Maintaining an erection becomes effortful more difficult.
● Erections every morning becomes less rigid or less frequent.
● Ejaculating or climaxing with an incomplete erection or more rapidly.

ED Treatment Options

Intracavernous Pharmacotherapy (ICP)

This is considered as a useful but underutilized treatment for ED. It involves using an auto-applicator to inject a combination of FDA-approved vasodilators into the specific spongy tissue of the penis. Hence, an erection occurs within minutes. In sporadic cases, the side-effects of ICP include pain or tenderness, penile bruising, scarring, and priapism. Boston Medical Group’s independent network of physicians had developed a particular approach to this method practical for thousands of patients. It is recommended for mn with heart conditions, diabetes, high blood pressure or other illnesses.

Oral Medications (ED Pills)

Oral medications are very well known, and the convenience they give make them the first-line of treatment. While these are effective for most men, those taking nitrates for chest pain should not use ED pills for the risk of an unsafe drop in blood pressure. Other side effects include but are not limited to, loss of hearing, blurred vision, and headaches.

Intraurethral Suppository (MUSE)

MUSE has a validator known as Alprostadil in the form of a pellet inserted into the urethra through an applicator.

Penile Prosthesis

A Penile prosthesis is often the last resort of treatment, given that there are other methods less invasive and more efficient. The best prosthesis is composed of a pump, inflatable tubes, and a reservoir. The inflatable tubes are inserted into the erectile chambers, the reservoir behind the pubic bones, and the pump in the scrotum. Upon activation of the pump, the reservoir allows fluids to flow into the tubes, which in effect hardens the penis and becomes erect. Bleeding, pain and discomfort, infection, and mechanical failure are some complications. Boston Medical Group physicians do not perform these surgical procedures.

Vacuum Suction Devices (VSDS)

A VSD is either a manually or battery-operated cylindrical pump fully enclosing the penis. Upon activation, the machine creates a vacuum by sucking the air out of the chamber, drawing blood to the penis. Some disadvantages include discomfort, incomplete erections, and the time required for operation.

Arterial and Venous Surgeries

About 2% to 5% of patients are possible candidates for surgery including venous litigation and arterial bypass. The risks are those related to other major surgical procedures. These operations are not performed by Boston Medical Group physicians.

Hormonal Replacement Therapy

Some men experience andropause, similar to menopause in women, that can be caused by low testosterone. Common symptoms are fatigue, poor sleeping habits, feelings of sluggishness, weight gain, little sex drive or erectile dysfunction. Hormonal Replacement Therapy can reverse these signs and even improve sexual drive.

Sex Therapy

Sex therapy is the usual treatment of choice for men with psychological ED. It requires the cooperation of sexual partner, multiple sessions and an experienced sex therapist for better results.

Sublingual tablets

These are for men who otherwise can tolerate the side-effects of ED pills but want a faster and more robust acting solution. These tablets are just as the same as oral medications, but because of speedier absorption, they have a quicker onset of response.

Premature Ejaculation

Premature Ejaculation (PE) is the prevalent form of male sexual dysfunction. In the simpler words, premature ejaculation is merely the ejaculation that happens before both partners would like in a majority of their sexual encounters. It may lead to stress, worry or concern. For some, PE may occur in all manners of sex, including masturbation. In other men, premature ejaculation materializes only during sexual encounters.

PE Symptoms

Uncontrolled ejaculation before or after penetration is the primary symptom of PE. It can happen in all sexual situations and during masturbation.

● Inability to control ejaculation.
● Ejaculating before or after he satisfied his partner.
● Ejaculationg before, upon, or after penetration.

PE Causes

Premature ejaculation is typically caused by both an over-sensitive glans penis and performance anxiety during sex. Biological causes of PE are:

● Thyroid issues
● Hormonal imbalances
● Abnormal reflex activity of the ejaculatory system
● Neurotransmitter abnormalities
● Inflammation and infection of the prostate or urethra

Adverse effects

The most common patient complaint is the stress that PE causes. It often leads to internalized guilt, lack of communication, and resentment.

PE Treatment

Premature ejaculation may seem like a natural, one-stop problem to solve, but in reality, it’s not. The endpoint in treating PE is to achieve satisfaction for both the patient and his partner.

Start and Stop Technique, Kegel Exercises, Squeeze Technique

The “Start and Stop” technique involves trying to delay ejaculation by either stopping the motion or withdrawing the penis just before the “point of no return.”
The “Squeeze” technique demands the partner to squeeze the head of the penis before the “point of no return.” These techniques require patience, consistency and unwavering support from the partner.

Kegel exercises, also known as pelvic floor exercises require repetitive contractions of the pelvic muscles. The result of this activity alone is limited, yet it can be enhanced with a combination of other treatments like ICP.

Local Anesthetic Gel, Spray or Cream

The purpose is to numb the head of the penis to lessen the penile sensation, therefore reducing the possibility of uncontrolled ejaculation.

Central Nervous System (CNS) Suppressants

Low Testosterone
